First there are 2 separate downloads of the extension for OSX are you sure you
downloaded the right one.One is for Version 4.0 the other for 4.1 and possibly
above.

Second Extensions are not created by the Apache OpenOffice project but by
individual users and are not supported by the project. You should contact the
creator of the extension if the proper download does not work.
