I think there's a possible solution about this problem: you can create using
for example FontLab a Font with all the letter underline. Give at it a
dedicate-name. Install your font into yours system. Modify FOP to handle the
new font and use it. This can be usefull.

Underlining isn't supported by FOP yet, so it's nothing you're doing.
