Extract forms from PDF

Dear Sir


          I want to use PDFBox to extract all forms from PDF file, but not found any samples from SVN and internet website.

         Can you kindly to help me on it?

What do you mean by "extract forms"? Extract the form data? Extract the
pages with the form fields? Extract the fields themselves (to another
file)? Please clarify.
