The issue can be reproduced with this file.

Adding a simple numbered cross-reference on OSX to a section after the current
position consistently crashes OO Writer.

Apparently it only happens on Mac OSX. I am currently experiencing it on 4.1.2,
but it also happened on 4.1.1. 

To reproduce, open the attached CrossRefOSX.odt file and add a Number-only
cross reference after the word 'my' to section 1.1. You will see that the
application crashes and exits.
