Successful queries that fall back to Original currently count as Calcite coverage. This also occurs with fallback_planner=none for unsupported features.

Record the actual planner and fallback reason. Count fallback and missing planner metadata separately, and add routing checks. Keep runtime fallback behavior unchanged.
