patch for XSDHandler

Hi,

Please find the attached patch for XSDhandler. Made a few more changes
to handle mutual <include>s. 

Previously, the XSDocumentInfo was added to fDoc2XSDocumentMap after 
returning from constructTrees(). I have changed the code to add it to
the map as and when a new XSDocumentInfo object is created. When a 
schema is included second time, it's XSDocumentInfo object (from 
fDoc2XSDocumentMap) is returned instead of constructTrees() being
called again.
