file does not begin with %PDF

I'm getting an error from a user that says when they go to look at a pdf
I've rendered they get a message that the file does not begin with %PDF.
Does anyone know what this means?  

I have embedded FOP in a servlet so I am returning the pdf supplying a MIME
type of application/pdf.  99% of my user population is fine.  Is this a
machine/Acrobat/IE issue?
