   A Markdown ATX heading below level one starts with ##, which Velocity reads
   as a line comment. In these *.md.vm pages every such heading is removed
   before Doxia sees the document, so the published page shows the text of each
   section with no heading above it.
   
   Level two headings now use a setext underline and deeper ones are wrapped in
   Velocity's unparsed block #[[ ... ]]#, both of which reach Doxia intact.
   
   Verified by building the site and checking the headings are present.
