If there are multiple moderators, then it's quite possible for two or more to send the unsub request. Only the first will succeed.

Since there is no reply to the second moderator, they have no idea what has happened unless they request the subscriber list.
Meanwhile the original subscriber gets an unexpected (and useless) message.
