The attached image is not displayed in the 1.8 versions, but is displayed in the 2.0 version. The PDF has this:

In 1.8, this is the AT just before displaying:

I'm aware that the graphic transforms are different in 1.8 and 2.0, but I still think that there must be some bug that has been fixed in 2.0 but not in 1.8, maybe related to the two 0 scale values in the ctm.
