[Redesign] layout update

The area tree now only contains the values used for rendering. The
min/opt/max values should only be handled by the layout managers and
when the areas are created and added then it can set the fixed size of
the areas. The area tree has also be cleaned up a bit to make better use
of the block parent.
Now need to sort out whether to use width/height or ipd/bpd for the area
tree and to make it consistent.

On the layout side the block now has some spacing but we need to sort
out a better way of handling breaks to make the correct spacing
adjustment.

The main layout things are sorting out proper break handling and working
out how to deal with footnotes/before floats and side floats.
