3.1.1-incubating upgrade docs

I just committed the first bit of upgrade docs for 3.1.1-incubating.  There
were a lot of additions already (at least from my end).  Didn't meant to
let it go for so long.  Probably should have rolled it into my PRs - that's
probably the best way to do this so that nothing is missed.
