Agreed. Something like, using language from the ICLA and License:
* I confirm this Contribution is made under either the terms and conditions of the Apache License 2.0, or a separate license agreement with the Apache Software Foundation (for example an ICLA or SGA). I also confirm that I have identified any parts of the Contribution that are not my original creation.
