AddDefaultCharset doesn't work with charset_name other then iso-8859-1. 
If I put AddDefaultCharset koi8-r in my httpd.conf I still get charset=iso-8859-1
in http headers.

Put AddDefaultCharset koi8-r in httpd.conf and look at http headers sent from server.

I tried to figure out this looking in the source code, but it seems OK at first glance.
