XML not well formed after transformation - xmlns:%@$#^@#="%@$#^@#"

Hi all,

What is someone doing wrong when, after applying a stylesheet to a
document, the result document contains xmlns:%@$#^@#="%@$#^@#".

The XSLT processor seems to add this namespace after a <xsl:copy-of
select="*"/> element. Anybody known with this behaviour?

Same problem here, with the Cocoon Forms stylesheets apparently.

Can you show more of the sample input, stylesheet, and output?
