form and perl cgi to manage dbm user database location (fwd)

Could someone check this out?  Doug, you might be interested in seeing if
you can merge projects, maybe....

The script header needs to have the correct apache header stuff put on, but
other than that, seems good to go.  Hope people find it useful.  It's
everything you need, just add perl, and it makes it's own gravy.
