Changes to fix the build for pdfdoc, mostly made to the xml2pdf.xsl stylesheet. 
Fixes 1) FAQ not displaying, 2) graphics not displaying, 3) bullets not 
displaying. Also, some general beautification work. Generally makes the pdf 
documentation more usable.

This patch is for the trunk.
