Add a Connection VTI

We should add a new VTI that lists all active connections and provides details about each connection, such as owning user, the associated client connection if there is one, etc.  Linked with the other VTIs this would be a very useful debugging tool.  One of the columns should be the unique connection string for that connection, once we have this available with DERBY-243.
