Yes.  I'm getting it under Linux.  It happens for both failed and
successful PTR lookups.

Looks like about 25% of or logs (from a sample of only two days) show
this.

Hmmm.  I see a couple here where the access to the HTML page shows
only the first part of the hostname, but the accesses of the GIFs on
that page all show the full hostname.
