New ZooKeeper wiki page on Troubleshooting

I added a new ZK page detailing how to troubleshoot the operating 
environment. A FAQ of sorts, it has a number of examples of situations 
we've seen and how to determine/resolve the issue. We'll expand on the 
content as new issues are seen.
