It sounds like the user profile has become corrupted to the extent that Apache
OpenOffice has concluded that the first-run request for user information has
not been done.

Note that registration is no longer required since movement to Apache
OpenOffice.  Is there some inhibition against upgrading to the latest release,
Apache OpenOffice 4.1.2?  

(Or perhaps registration is a confusing wording.  What I mean is that no
registration information is communicated to or collected by the Apache
OpenOffice project.  "Registering" user profile information allows it to be
included in fields and properties of documents being created, but it applies to
the local computer account.)

A common cause for profile corruption is a hasty shutdown, whether because of a
computer crash or not closing all open documents and then waiting a little
while before shutting down the computer.
