Attaching patch 1a, which removes the HTML Book entries from the Derby documentation HTML page.

I think this is ready for commit, but what's the conclusion on the appropriate action to take?
Given that we have PDFs in addition to the HTML pages version, shall we err on the side of caution and remove the HTML Book version?
