Subject: fixed up mod_auth_mysql


For those of you using MySQL HTTP authentification.  I've updated the
mod_auth_mysql-1.3w module, adding support for the username and password
args to mysql_connect via the Directory/.htaccess configuration in Apache.
Also cleaned up a bunch of compile time warnings.
