First problem: You're creating DOM Level 1 non-namespaced nodes. XPath and XSLT
require namespace support. Replace all DOM calls to createElement/createAttibute
with createElementNS and createAttributeNS. If the problem persists after you've
made that correction, let us know.

(The DOM Working Group would have deprecated the old calls, but there is no
mechanism in the W3C for that sort of deprecation.)
