Derby (JavaDB) Dialect for DBAppender

Hello,

Below is the source for a Derby implementation of the
org.apache.log4j.db.dialect.SQLDialect.java interface.  Tested and
working with Derby 10.2.  The enumeration of dialects in
org.apache.log4j.db.dialect.Util.java needs to be updated to support
the new dialect.
