Win32 naming conventions

One of Win32's more obnoxious habits is that once a DLL is loaded, it forgets
about paths, etc., and just uses the DLL's filename. This means that it is
important to have DLL names that don't clash. I'd suggest, therefore, that we
start all Apache DLLs with "Apache", and modules with "ApacheModule", or
similar.
