outputting DOCTYPE declaration

I'm wondering if I have a DOM output from after I apply the transform,
how do I stick in a DOCTYPE declaration?  Can I do it in the
transformation phrase, or can I do it by inserting into the DOM
directly?  Some pointers would be greatly appreciated.
