Fix Version/s: 2.12.0

> Issue Type: Bug
> Components: DOM (Level 3 Core)
> Affects Versions: 2.11.0

> When the "namespace" parameter is true and the "namespace-declarations" parameter is set to false a call to Document.normalizeDocument() is supposed to discard namespace declarations. This often works, but there are a few cases where Xerces fails to remove namespace declarations from an element node.
