I had been trying out test file
https://home.apache.org/~mseidel/AOO-builds/AOO-4111-Test/Full%20Installation/Apache_OpenOffice_4.1.11_Win_x86_install_en-US_f2fa887bab.exe
under issue 128453 concerning .wpl files.
On returning to inserting a .txt file into a slide AOO crashed due to
unexpected error. Both the Impress file I was working on and another which was
open both had to be recovered. I retried the insertion and the same thing
happened again.

Please test again with AOO 4.1.10.

I doubt that this is related to the 4.1.1 Test build.

This is a new fault which appeared when using the test 4.1.11 file. It concerns
a crash when inserting a .txt file and affects an open writer as well as
impress files.
Issue 128462 related to .odt file being inserted so is not related.
I need the system working tomorrow so will have to revert to 4.1.10.

I had open an Impress file with an empty slide selected.
I clicked Insert > File -> Explorer opens (now Windows style, previously AOO
style).
I selected any .txt file > Open -> crash windows shown in attachment.

Before testing file in the original description this did not occur. It started
after the test without returning to 4.1.10. Now it occurs every time on 4.1.10.
No change in procedure or file. No change with new profile.

As you will have seen there the new test 4.1.11 cured the crashes.
Only outstanding is issue #128462.

I'm back from village commitments. The two issues we are on require different
versions of OOo. 
Here I'm on v4.1.10. I've tried remaking the profile as suggested and
everything hung up, even the Start button failed to open to provide a restart.
Eventually a right-click in open space on the desktop woke the machine up. I
restarted and cleared the new OpenOffice profile file and started Impress.
Insert file crashed as before. I noticed that the file browser was the standard
Windows one not the AAO which I had got used to. Attached crash report shows
two windows which actually overlapped. Recovery then followed as expected.
Copy and paste the text file worked.

That's odd, since that build doesn't change anything in that respect.
It only adds m4a to the list of secure extensions.

I am sure that the renaming of the profile went wrong. Otherwise I should be
able to recreate your crash.

Please reboot your PC and (as the first) action, rename the OO profile.

It's back to 4.1.10 and still crashing. I've been inputting .txt files to
Impress slides for weeks. No idea what is wrong and I have to have it working
Sunday morning.

It's crashing again!
New ODP not from my template.
Insert > File > select plain TXT file -> OpenOffice Document Recovery.
Tried several times, on one occasion I had to run the recovery twice to get the
ODP open.

I rebooted, deleted the profile, opened Impress so a new profile was created,
tried to insert a file but it still crashed.

And you are sure that there was no process open from the "disappeared" Impress
file?

Maybe someone else can recreate the crash? I can import a text file into an
empty Impress slide without problem.

I cannot reproduce that.

The Windows Explorer is default, you have to change it in Tools -Options if you
want the OO dialog.

That's what I expected.

Try renaming your OpenOffice profile. In most cases that helps.
