new look for Rivet manual

The manual with the new CSS file has been copied to

 http://people.apache.org/~mxmanghi/manual/

The relative size of characters is not satisfactory, but you can have an
idea of how the manual would look like. I thought I could have a better
balance but the way the rules in the CSS apply to the page elements
doesn't seem trivial and needs some work.

navigation icons were recreated using a vector graphics editor.
