Extended manual and other recent commits

I've just commited the changes made to rivetCore.c and TclWebapache.c 
in
response to the issues with makeurl, load_env and load_headers Jeff
Lawson brought up on bugzilla. I will soon also close those bugs.

I also commited a new commands.xml files. Commands are now listed in
alphabetical order and pages for already existing undocumented
commands were added.

Alphabetical order is better when you know what you're looking for,
but it would require an index of references grouped by functionality
and purposes. After all the command set is still small enough that
the list is easily readable, even when a search based on a command's
purpose is done.

English native speakers are welcome to read the new pages and check
them out for anything wrong or improper I surely wrote.
