updates to the committers/subscribe tool

https://whimsy.apache.org/committers/subscribe

For starters, this tool wouldn't work on whimsy-vm4. It kept saying:

svnmucc: E160016: Can't commit to
'https://svn.apache.org/repos/infra/infrastructure/trunk/subreq' because
it is not a directory

Well, that is a directory, so all I can think of is that the version of
svn on whimsy-vm4 (1.9.3) has a bug in it that the version of svn on
whimsy-vm3 (1.8.8) does not. Or that the syntax of svnmucc command
changed in a non-backwards compatible fashion - but I could find no
evidence of this.

So I change the single svnmucc command to three commands: svn checkout,
svn add, and svn commit. That sequence should work on both machines.

Other changes:

1) I checked the return code from the commit and produce a different
alert text box depending on the result.

2) I moved the form so that it was always at the top. Previously it
would move to the bottom after a request was submitted.

3) I changed the list selection to be a live search. For me, there
seems to be a momentary pause when you first click on it, but after that
it is much easier to use than a humongous list.

If people spot any problems, let me know.
