I did not have this bug in earlier OO versions and I had it for quite some
time, but only right now I got a better idea, of what actually happens. It
seems to happen only occasionally, that means, I do not know under what
conditions it occurs. 
The error is simple, but I've chosen severity 'major', because the text in the
document is corrupted by it, often unnoticed. Really troublesome. 
What happens? Very simple. Have a text and replace some text by copy and paste
via the clipboard and the key shortcuts (C&V). The error is, that it gets
sometimes pasted twice. 
I noticed it only better now, because I have two footnotes (right behind each
other - but it does not happen only in footnotes). Both contained the text to
be replaced by copy and paste and after replacing the text in the 2nd footnote,
it added a 2nd paste in the 1st footnote right after the replacement, something
like: 

footnote 1: ... REPLACEDREPLACED ... 
footnote 2: ... REPLACED ... 

It seems, it happens also at the position where the cursor has been before.
Therefore sometimes you find the accidental replacement also at the very
beginning of the document.

I tested this bug on Windows 7 Professional Service Pack 1 build number 1130. I
also tested this bug using OpenOffice 4.0.1 and 4.2.0. I was able to replicate
this bug on both versions, but under one condition not specified by the
original poster. I had to select the text to be replaced using multiselection
(hold CTRL while clicking on text to be replaced). The steps I followed to
replicate the bug are as follows:

1. Open blank OpenOffice Writer document
2. Type two words, separated by a space ("Blah Blah")
3. Highlight Blah, and copy to clipboard (Press CTRL+C)
4. Multi-select both "Blah"'s (click on them while holding CTRL)
5. Paste Blah from clipboard (Press CTRL+V)
You will notice either "BlahBlah Blah" or "Blah BlahBlah", the second Blah will
be appended to whichever "Blah" you selected last. 

The only way I could get this bug to work was following the steps above,
otherwise it would just copy and paste once. For example if I:
1. Open blank OpenOffice Writer document
2. Create a line of text.
3. Copy the text to clipboard (CTRL+C)
4. Paste the text from clipboard (CTRL+V)
No matter where I paste the text, if I am not using multiselect than it only
pastes once.

... both bugs might have (a) common root(s), but the other report seems to be
quite different. 
I cannot yet manage to create a document, which always shows the bug ... :-(

Incidentally I noticed more of the wrong behavior. 
Imagine you have 2 footnotes right behind each other and both have the string
to replace, for example 'TOREPLACE'. When you search for it, the string is
marked. If I replace it goes wrong. If I add a letter (for example just 'x')
behind TOREPLACE, the paste avoids the problem. 
Again, if I have the 2 footnotes ... If the replacement works fine for the
first footnote, when doing this for the 2nd footnote, the erroneous adding is
done for the first footnote ... but: If I do an Undo both insert disappear,
another paste works without the error. 
Therefore: The Undo knows about 2 operations!

Unfortunately, I have not been able to replicate the bug.

I tried to replicate it with both 3.4.0 and 3.4.1 versions of Open Office. The
operating system is Mac OS X Mountain Lion 1.8.2

My steps for each version were:

1) Uninstalled any previous version of AOO before installing another version.
2) Opened AOO Writer
3) Opened New Document
3) Added content (two paragraphs) to the document along side with two
footnotes.
4) a)Without saving: Copied selected text via CTRL-C and CTRL-V and pasted it
over  different places on the document, including within paragraphs and
footnotes. The text was pasted over other selected text or after text. Later
tried to copy and paste with the the menu buttons using the same procedure.
Finally, used combinations of keyboard shortcuts and buttons to copy and paste
text. No duplicated text was generated.
  b)Saving as an .ODT file: First saved the document, and then repeated the
same procedures as done without saving. No duplicated text was generated.
5) Closed the document.
6) Reopened the document.
7) Repeated the same procedure for copying and pasting. No duplicated text was
generated.
8) Closed the document.
9) Quit AOO.
10) Restarted AOO.
12) Reopened the document.
13) Repeated the same procedure for copying and pasting. No duplicated text was
generated.

A text document with the similar characteristics as reported by the bug report.

This .odt text document has some content(two paragraphs) and two footnotes in
order to test the bug, as suggested by the person who reported the bug.
