How to add a button on UI

Hi, here's another query:
We wish to add a menu item below the "New Document" menu item (in the File 
menu in the authoring view) that links to an executable file that performs 
some functionality we need. How do we achieve this? Plz explain how to achieve 
that 'linking to the executable' aswell.
