ensure that all html form intputs have proper labels with them

There are a few places that do not have html labels on the forms.  This makes it harder to select choices (in the case of checkboxes or radio buttons).

Some examples:
addProxyConnector.action - the policy dropdowns
repositories.action - "Process All Artifacts" checkbox text
repositoryGroups.action - "Identifier*:" add groupd
repositoryScanning.action - the checkboxes for the "Repository Scanning - Consumers of Known Content"
index.action - all fields on the search and advanced search
queryAuditLogReport.action - all fields
