The file in doc format (here you see the bug)
To reproduce:
- load the doc file in LibreOffice
- go to the index and edit it
- set index depth to 3: now you should see the subchapters in the index
- save the file in the same format
- close the file
- reopen it: you should see that the index is limited to depth 1
If I save in odt format I do not see the bug.
This happens to me on LibreOffice 3.4.3 on Debian 'wheezy'.
It has has been reproduced with OOo 3.3.0 on Vista.

There are only two levels in attachment 76956, index is not limited to level
one.
