Dont qoute me, but I think you use
servername domainname.com
serveralias www.domainname.com

Yes, You definitely should have it in the DNS, otherwise it will not
resolve.

Well, provided that your DNS has been setup then fine... otherwise, I don't
think Apache will recognise it without the www
