Because the plugin.xml lacks `uses-permissions` for `CAMERA` and `READ_EXTERNAL_STORAGE`, once a user has denied camera access always, they cannot undo it, even in the permissions setting for the app.

The only workaround is uninstalling/renstalling the app or manually modifying the AndroidManifest.xml to include

```
<uses-permission android:name="android.permission.CAMERA" />
<uses-permission android:name="android.permission.READ_EXTERNAL_STORAGE" />
```

so they can be modified in the application manager settings.
