On the example page above, the url field is listed, but there is no description of the format the string should take.

For example, for iSCSI, it should take this format:  iscsi://<ip address or host name>/iqn/LUN

Several protocols are supported here in CloudStack (examples:  nfs, vmfs, iSCSI, shared mount point), so each one may have a different format.
