ap_cache header file

I am really trying to document all of the API's in Apache 2.0, but I have
hit a brick wall.  Could whoever wrote all of tha ap_cache stuff please
add docs to it.  I would prefer scandoc format, because that means I don't
have to go back in a re-format all the docs, but at this point I will take
whatever format you prefer.
