When I removed him from the issues@ ML, I also check all the other MLs and that mail address was not subscribed.
If you still have an issue, please forward the entire message, along with all the mail headers.
I am closing this now, as I think this is complete and you were seeing a delayed artefact.

Thank you for your help. I tried removing the user from dev@stdcxx.apache.org yesterday but the command failed (user not subscribed). The issue persists even today (there was just a new moderation request for the same user in the dev@ queue). I also looked at the log but didn't see anything out of the ordinary.
Is there anything else you would suggest to try?

I saw Tony's comment about having removed the address from issues@ but then saw a moderation email for a post to dev@ (or commits@, I'm not sure I'm reading the emails right), neither of which has the address as a subscriber. Maybe I'm also not translating the time zones quite right (I'm travelling, on Hawaii time with my laptop still on Mountain time). If that's the case I apologize in advance for my confusion. Let me let things settle for a bit and see if the problem comes back.
