Missing description of how to deploy an SCA WS with Apache HTTPD and mod_axis2

The SCA WS documentation should include the steps to deploy a Web Service using Apache HTTPD and mod_axis2.

I don't think this is really a release blocker but we should get this documentation on the Web site at least shortly after the release.

    Resolution: Fixed

Verified that the integration with HTTPD works (had to fix a few bugs in our code) and added a sample (HttpdBigBank) with a minimal HTTPD configuration and instructions to start the server and verify that it works.
