[FIXME] Default pub / publication-sitemap.xmap

In the above file you will find at lines 85-90 that the parameter "url"
is not declared in page2xhtml.xsl.

The other way round page2xhtml declares a param "document-id" which
is,imho, never passed.

Maybe someone should fix it.

BTW: I propose to hand all 3 params to page2xhtml so,
root,url,docuemnt-id.

I would fix it myself, at least the first thing, but ..... cvs doesn't
like me, yet.
