I've published at http://examploforms.org an initial draft of a new open

source authoring/modeling tool for forms.

Think of Schematron and Examplotron, applied to form properties like 
required, read-only, calculations, etc. It includes a stylesheet that 
transforms Examplotron documents into XHTML+XForms. The focus for the 
initial release is not to produce beautiful-looking forms, so if you 
play with it, be prepared to spend some quality time in 'view-source' 
mode .
