Enable extlink extension. Setup extlink to CouchDB JIRA and Apache Git commits.

This extension allows to create markup-based URLs that points to specific location.
For instance, use :issue:`1234` to let sphinx create a direct link to COUCHDB-1234 issue on JIRA tracker.
