Tolga,
First please note the mailing list will delete any attachments, use a pastebin service instead.
Secondly, have you installed on VirtIO devices when you prepared your template?

When i create new template i selected Windows PV or Other PV 64 bit as you
recommend. But i saw an error page.
"Windows failed to start. A recent hardware or software change might be the

Try to add it with "Windows PV" or "Other PV 64bit", the latter will definitely give you PV (VirtIO) disk and network.
