ASF license 2.0

Just re-read http://www.apache.org/dev/apply-license.html about how to apply
the license, and it includes:

Yes. All source files should include the copyright notice boilerplate that
indicates it is owned by the ASF and that it is distributed under the Apache
License, Version 2.0.

This probably means we also need to apply the license to other source files
such as:
- batch and shell scripts
- properties files
- sample JMX files
- build script

I think (hope!) we may be allowed to omit the license for test data
files...!

Oh, and I presume we also need to update license.xml and re-publish the
web-site...
