If I understood it correctly,
PLAIN_DICTIONARY: 1 PLAIN encoded Dictionary page with the dictionary values followed by rle-encoded Data pages with corresponding indices  (4 byte integers).
RLE_DICTIONARY: rle encoded Data pages with indices (4 byte integers). (There is no dictionary associated)
