I have found out what caused this. Here is a fragment from web.xml

<ejb-local-ref id="EJBLocalRef_1115652762940">
		<ejb-ref-name>ejb/UnderwritingFacadeLocal</ejb-ref-name>
		<ejb-ref-type>Entity</ejb-ref-type>
		<local-home></local-home>
		<local></local>
</ejb-local-ref>

As you can see the local-home and local entries are blank.
