Can anyone point me to some documentation that is specific to getting
the Jakarta ISAPI plugin to work with IIS 6.0 on Windows Server 2003?

This all seems to work properly on Windows XP (IIS 5.0), but I can't
seem to get it to work on Windows Server 2003 (IIS 6.0).

The most important thing is that after adding the filter to the
'Default Web Site' (like for IIS 5.x) you will need to add
the Web Service Extension.

Click on 'Web Service Extensions', and then click on
'Add a new Web service extension...'
Type 'TomcatRedirector' under Extension name (or
what ever you prefer), and click 'Add' and browse for
the installed isapi_redirect.dll.
Check the 'Set extension status to Allowed' check box.

That's it.
There is no need to go to the IIS5 isolation mode.
